Dear Parents and Guardians,
If you qualify for bus transportation, transportation requests forms are due in the South Loop School office by Friday, June, 3. See the attachment for a copy of the "White Form". If you need further information, or more forms, such as sibling requests forms, please do the following:
- Go to www.cps.edu
- Click the "Programs" link at the top
- Select "Wellness and Transportation"
- Select "Transportation"
- Select the PDF form you need.
